At the Disability Resource Center, we believe that everyone deserves the opportunity to live independently in the community of their choice. Yet many people with disabilities and older adults find themselves living in nursing homes—not because they want to, but because they don’t know there’s another way.


Whether you’re currently in a long-term care facility, in short-term rehab, or facing the possibility of institutional placement, DRC is here to support your right to choose a more independent future.

Our Nursing Home Transition Program helps individuals with disabilities transition from nursing homes back into the community. We also assist those at risk of institutionalization in finding the resources they need to remain in their homes and avoid future admissions.

Our services are guided by your goals, your preferences, and your timeline.

Your DRC Transition Coordinator will work with you to access the supports you want—not just what’s available. From housing and home modifications to in-home care and benefits navigation, we’ll help you create a plan that fits your life.

Because it is your life—and you should be the one to decide how and where to live it.
